LEAVE APPLICATION FORM
Terms and conditions:
1. This form is applicable for foreign employees only.
2. The LAF process must be initiated within 60 days from date of submission. Any changes that incur cost will be borne by the foreign employee.
3. Maximum approved duration for long leaves is 2 continuous months in a year. Leave application closed to 60 days of permit expiry date is not allowed.
4. Extend leave will be considered as unpaid leave.
5. Approval for long holidays is subject to HOD and Plant Manager.
6. The HR Dept will facilitate the purchase of affordable round trip air tickets with the consent of foreign employee.
7. The HR Dept will arrange free transportation for foreign employee from hostel to airport. However, returning foreign employees is responsible to arrange their own transport from the airport to the hostel with their own expenses.
8. No guarantor is required. Foreign employee must provide sufficient cash for the purchase of air tickets.
9. Employee are required to return to Malaysia according to the ticket date of purchase. Failure to return will be considered as absconded.
10. It is discouraged to approve long holidays if the HOD has plans or decided to discontinue the employee when his permit expired.

RENEWAL PERMIT NOTIFICATION FORM
Purpose:
1. This notification form must be submitted to the HR Department within 90 days from the expiration date of the work permit.
2. This notification serves solely for administrative purposes and is not intended as a means to negotiate salary increments or promotions with superiors. Any requests for salary increments or promotions will be handled through the appraisal process.
3. Information provided must be accurate to prevent the submission of false information.
Employee’s Responsibility:
1. Employees are responsible for renewing their passports at their own expense in accordance with HR arrangements.
2. Employees must decide whether to extend their stay for another year or depart upon permit expiry.
3. Any changes in decisions will not be entertained once the notification form is submitted. Any subsequent changes will incur costs borne by the employee.
4. Employees opting to renew their permits must undergo a medical check-up. Any appeals will be the financial responsibility of the employee.
HOD’s Responsibility:
1. Head of Departments (HODs) may choose to support employee’s decisions. In cases of conflict, please involve HR for further discussion.
