Powder Mask failed to maintain proper 5S housekeeping and hygiene standards at the production area, which led to poor environmental control. This lapse was identified following a customer complaint involving a foreign contaminant (insect) found in the finished product, indicating that inadequate housekeeping practices may have contributed to the quality non-conformance.
This conduct is viewed as a breach of the Company’s policies on:
1. 5S Housekeeping Policy – Failure to maintain required workplace cleanliness and orderliness standards.
2. Good Manufacturing Practice (GMP) Policy – Non-compliance with hygiene, sanitation, and environmental control requirements to prevent product contamination.
3. Quality Management System (QMS) Procedures – Failure to adhere to documented procedures for maintaining a controlled and contamination-free production environment.
