The employee failed to ensure proper and thorough cleaning of the powder mixing tank, particularly at the bottom valve area, which likely led to foreign material contamination in two finished product jars and resulted in a customer replacement cost.

This conduct is viewed as a breach of the Company’s policies on:

1. Cleaning and Sanitation / Equipment Cleaning Procedure – Failure to ensure thorough cleaning of the mixing tank, especially at high-risk areas such as the bottom valve.

2. Good Manufacturing Practice (GMP) / Quality Policy – Inadequate hygiene control led to foreign material contamination in finished products, compromising product quality and compliance.

and is considered a matter of serious concern.

You are hereby required to provide a written explanation to justify why disciplinary action should not be taken against you for the above-mentioned misconduct. Your explanation must reach the HR Department by 3 working days which is
