December 23, 2025

During the filling operation, a bug was discovered in the product. While the filling process was eventually put on hold and further checks were conducted, it has been noted that the quality issue was not reported immediately by the employee upon discovery. Delayed reporting of such issues poses a potential risk to product quality, consumer safety, and compliance with GMP requirements.
This conduct is viewed as a breach of the Company’s policies on:
1. Failure to Immediately Report a Quality Deviation
Non-compliance with the Deviation Management / GMP Reporting Procedure by not promptly reporting the discovery of a foreign matter (bug) during the filling operation.
2. Breach of GMP and Product Safety Responsibilities
Failure to uphold GMP requirements by delaying escalation of a quality issue, thereby posing potential risks to product quality, consumer safety, and regulatory compliance.

You are hereby required to provide a written explanation to justify why disciplinary action should not be taken against you for the above-mentioned misconduct. Your explanation must reach the HR Department by 3 working days which is
