August 27, 2025

The colour discrepancy was not flagged during the employee's QC inspection. This has caused the customer to complain that foundation colour code Creme Brulee not same as previous batch SOR240020. When end user receive it, they made a complain to MAAEZ HQ that the current foundation colour code is more darker compared to previous one.
This conduct is viewed as a breach of the Company’s policies on:
1. Good Manufacturing Practices (GMP) – Quality Control Inspection Requirements
Failure to perform adequate visual and colour consistency checks during product inspection.
2. Quality Control Inspection and Documentation Procedures
Failure to detect, record, and escalate product non-conformities, resulting in inaccurate quality records.
and is considered a matter of serious concern.
You are hereby required to provide a written explanation to justify why disciplinary action should not be taken against you for the above-mentioned misconduct. Your explanation must reach the HR Department by 3 working days which is

If you fail to submit your explanation within the stipulated time, the Company will proceed to make a decision on the matter based on the available information, which may include disciplinary action up to and including dismissal.
